Documents for Regular and Associate Membership

• Accomplished membership application form.
• TIN ID and two valid IDs, such as UMID, SSS, driver’s license, passport, voter’s ID, or postal ID.
• Proof of address: water, electricity, or internet billing statement.
• Proof of income: valid business permits, certificate of employment or payslip, or OFW remittance slip, as applicable.
• Colored photos: two 2×2 photos and one 1×1 photo.
• Marriage certificate if married, or birth certificate if single.

Regular membership application steps

1. Secure and complete the membership form and submit it with the required documents.
2. Undergo a credit and background investigation.
3. Attend the Pre-Membership Education Seminar (PMES).
4. Arrange share capital placement and complete the depositor information and specimen signature card, membership agreement, and Damayan Savings Account enrollment form.
5. Deposit the required investments and payments at the counter.
6. Collect your Savings Deposit passbook before leaving the counter, then return to the Member Relation Assistant (MRA) for your Share Capital passbook.
7. Wait at least one week from share capital placement before loan availment.

Submitting the online form starts your application; it does not confirm membership or loan approval. Contact staff for document review, the PMES schedule, and the remaining steps.
